The Township's Report a Concern Form goes beyond simply submitting your concern or feedback. By using the Report a Concern Form, your issue is entered into a case tracking and management system allowing staff to see the issue and its details seconds after the issue is submitted. As long as you provide an email, you will be notified when your issue has been received, when it has been updated, and when it has been resolved.

Questions about how the Report a Concern Form works?
The Form acts as an intake for your issue. At the time of reporting, you are encouraged to complete the form with as much detail as possible. Fill out your contact info at the start of the form so that you can receive updates as they happen and be notified when the issue has been resolved. The form allows you to set an exact location, description, and attach any photos.
Not sure what issues can be reported through the system?
The Form allows you to select the appropriate category for your issue. This ensures that the issue is sent to the correct department. If the issue you are reporting does not fall under any of the available categories, report "General Inquiry".
