Finding a primary care provider is an important step toward better health — and you don’t have to do it alone. If you live in Grey or Bruce County and need a family doctor or nurse practitioner, Health Care Connect can help match you with a provider in your community who is accepting new patients.

How to register:

• Online: Complete the form at ontario.ca/healthcareconnect

• Phone: Call 8-1-1, available 24/7

Once registered, a Care Connector from Health Care Connect will work to match you with a provider as soon as possible. You can sign up even if you are already on a local waitlist — it’s one more way to get connected to care.
